Some of the best dermatology care starts with someone who knows how to listen, what to notice, and when to take a closer look.
This National Physician Assistant Week, we’re celebrating the PAs who are such an important part of dermatologic care.
Their impact reaches far beyond the exam room. Through clinical expertise, thoughtful collaboration, patient education, and genuine connection, PAs help strengthen our care teams while helping patients feel informed, reassured, and cared for.
From evaluating skin concerns and managing ongoing conditions to answering questions and guiding treatment conversations, their work makes a difference every day.
To the PAs who care for our patients and work alongside our teams: thank you for your expertise, partnership, and commitment to exceptional patient care.
Happy PA Week. 🤍

If your melasma keeps coming back, there’s a reason.
And it probably isn’t because you “did skincare wrong.”
Melasma can be frustrating.
It fades.
It flares.
It improves.
Then somehow… it shows up again.
That’s because melasma isn’t just a simple dark spot.
Sun exposure can play a role.
So can visible light.
Hormonal changes may influence it.
Genetics can matter too.
And irritated skin can make pigmentation look even more noticeable.
So what can you actually do?
Start with the basics.
Wear broad-spectrum SPF every day.
For melasma, a tinted sunscreen with iron oxide can be especially helpful because it adds protection from visible light.
Reapply when you’re spending time outdoors.
Use shade, hats, and sunglasses when you can.
And pay attention to how your skincare feels.
Burning and stinging are not signs that a product is “working harder.”
If your routine constantly leaves your skin irritated, it may be time to simplify.
Gentle, consistent skincare usually beats an aggressive routine you can’t tolerate.
And if you’ve been buying brightening product after brightening product without seeing the change you hoped for?
That’s where medical dermatology matters.
A dermatologist can look at the pigmentation itself.
Consider possible triggers.
Review what you’re already using.
And help determine whether prescription skincare, professional treatment, or a different daily routine makes sense for your skin.
Because treating melasma isn’t about throwing everything at it.
It’s about having a plan.
If stubborn facial pigmentation keeps returning, stop guessing.
Book a dermatology appointment and let’s build a treatment plan around your skin, your history, and your goals.

Hyperpigmentation can feel frustrating to manage, but understanding the root cause makes all the difference.

Whether dark spots stem from sun exposure, old acne marks, or hormonal shifts, treating them requires a targeted approach and plenty of sun protection to keep them from returning. Swipe through to learn what causes excess pigment and how dermatologists safely brighten skin.

If your “dry skin” keeps coming back, it may be trying to tell you something.
October is Eczema Awareness Month.
And eczema is so much more than a little dryness.
It can itch.
Burn.
Feel tight.
Flare without warning.
Settle down.
Then come right back again.
For millions of people, that cycle becomes part of everyday life.
But persistent irritation should not automatically become your “normal.”
Eczema, including atopic dermatitis, is a chronic inflammatory skin condition.
And it doesn’t always look the way you might expect.
On some skin tones, eczema may look pink or red.
On others, it can appear brown, purple, gray, or darker than the surrounding skin.
That’s one reason seeing a dermatologist matters.
Medical dermatology looks beyond what’s visible.
We look at your symptoms.
Your flare patterns.
Your skin barrier.
Your possible triggers.
And how your skin is affecting your day-to-day life.
If you’re dealing with recurring itchy, dry, irritated skin, start with the basics.
Use a gentle, fragrance-free cleanser.
Moisturize consistently.
Avoid very hot showers.
Pay attention to products or environments that seem to trigger a flare.
And resist the urge to keep switching products every time your skin reacts.
Sometimes barrier care helps.
Sometimes moisturizer simply isn’t enough.
That’s when an individualized eczema treatment plan can make a real difference.
And one more thing worth saying:
Eczema is not contagious.
It is not “bad skin.”
And it is nothing to be embarrassed about.
You deserve answers.
You deserve a plan.
And you deserve skin care that takes your symptoms seriously.
If itching, irritation, or recurring rashes keep coming back, make an appointment with a dermatologist.
Don’t spend another season guessing what your skin needs.

What people think Moxi® is 🚫 vs. what it actually is ✅

Myth: It’s a scary laser with weeks of downtime.

Reality: It’s 15 minutes in the chair and back at your desk by 1 p.m.

The Moxi laser improves overall skin tone and texture, reduces pigmentation and smooths fine lines, with minimal downtime.

✨ Here’s how neurotoxins help: When a muscle is paralyzed, it moves less. Over time, that muscle gets trained to relax, since it isn’t repeating the same motion anymore. That means fewer lines have the chance to form in the first place.

It isn’t just about smoothing what’s already there; it’s about preventing what’s next.

Your rash finally shows up.

Then disappears the day before your dermatology appointment.

Sound familiar?

Skin has terrible timing.

Rashes can flare.

Fade.

Spread.

Change color.

Move to a new area.

Or look completely different by the time you’re sitting in the exam room.

That doesn’t mean the flare didn’t matter.

It means your dermatologist may need a little help seeing the full story.

One of the simplest things you can do?

Take photos while it’s happening.

Not just one super-zoomed-in picture.

Try the 3-photo rule:

📍 WHERE
Take a wider photo that shows where the rash is located on your body.

🔎 AROUND IT
Include some surrounding normal skin so there’s context and comparison.

📸 DETAIL
Take a clear close-up in natural light.

Skip filters.

Skip beauty mode.

And try to keep the image sharp and true to what you’re actually seeing.

Then make a quick note.

When did it start?

Does it itch, burn, hurt, or feel tender?

Has it spread or changed?

Did you start a new skincare product, medication, detergent, or topical treatment?

Did anything make it better—or worse?

Those details can give your dermatology provider a much clearer picture of what happened between flare and appointment.

Medical dermatology is often about patterns.

What your skin looks like.

Where it appears.

How it changes.

And what happened around the time it started.

So the next time your skin acts up, don’t rely on memory alone.

Document it.

Then bring the story with you.

If you’re dealing with a persistent, recurring, painful, spreading, or unexplained rash, don’t keep guessing.

Book an appointment with our dermatology team so we can evaluate what’s going on and help you find the right next step.

More than 70 million Americans rely on Medicare for access to health care, including life-saving skin cancer screenings and treatment.

Today, that access is at risk.

Proposed Medicare payment cuts could make it much harder for patients to access dermatology care, including skin cancer screenings, diagnosis, and treatment.

And when it comes to skin cancer, early detection can mean the difference between life and death. For someone worried about a suspicious spot or waiting for answers about a potential skin cancer, any delay can have serious consequences. We cannot afford to put early detection and treatment at risk when patients need it most.

Forefront Dermatology, behind our passionate President, Dr. Betsy Wernli, is actively advocating for Medicare patients and the physicians who care for them. But we need your voice to make a bigger difference.

Read on to find out how you can take quick action to help fight for access to life-saving care: https://forefrontdermatology.com/medicare-cuts/

Ever wondered what causes those tiny bumps on your skin? 🧬

Skin tags often form in areas where skin rubs against skin or clothing, but they can also be linked to genetics, hormonal shifts, or metabolic changes like insulin resistance.

While they aren’t dangerous, attempting to remove them at home with string, shears, or vinegar can lead to scarring and severe infection. Safe, sterile removal options, such as professional cauterization or cryotherapy, ensure quick results with minimal to no downtime.

Toxin consultations aren’t one-size-fits-all. 💉

Ashley Dietrich, MD, FAAD walks through how she customizes a treatment. She uses it in the glabella and crow’s feet to soften expression lines, in the eyebrow for a subtle lift, and under the eye to help open and brighten that area.

Every part works together, and that’s the point.

Your skin is not asking for a perfect diet.
And no, one “skin superfood” is probably not going to fix everything.
That may be less exciting than the advice you see online.
But it’s a lot more useful.
Because healthy skin is influenced by the whole picture.
Nutrition.
Sun exposure.
Your skin barrier.
Hormones.
Genetics.
Medications.
Your skincare routine.
And sometimes an actual medical skin condition.
So what does food really have to do with your skin?
More than nothing.
But less than social media sometimes makes it sound.
Drinking enough water supports normal hydration.
But if your skin stays dry, your skin barrier may need more attention than your water bottle.
Diet may influence acne for some people.
But persistent acne can also involve oil production, clogged pores, inflammation, hormones, genetics, and medications.
Collagen?
Your body absolutely needs the nutrients involved in building and maintaining healthy tissue.
But collagen powder cannot selectively tell your body to rebuild the collagen in your face.
And when it comes to visible skin aging, one everyday habit deserves a lot more attention:
Daily sunscreen.
A few skin-health basics worth keeping simple:
Eat a varied, nutrient-rich diet.
Stay adequately hydrated.
Wear broad-spectrum sunscreen every day.
Use skincare that supports your skin instead of constantly irritating it.
And pay attention when something changes or refuses to improve.
Because food can support healthy skin.
It does not replace medical dermatology.
If you are dealing with persistent acne, dryness, irritation, pigmentation, hair loss, a changing spot, or another skin concern, you do not have to keep guessing.
A dermatologist can help you figure out what is actually happening with your skin and build a treatment plan that makes sense for you.

Starting anti-aging treatments early isn’t about changing how you look, it’s about protecting your skin’s future.

Preventive care focuses on maintaining collagen production, smoothing dynamic fine lines before they set, and keeping early sun damage at bay.

By incorporating targeted treatments like light lasers, gentle peels, and preventative neuromodulators in your 20s and 30s, you support your skin’s natural resilience over time.

Your manicure might be covering something worth noticing. 👀

Most of us look at our nails and think:

Polish color.
Length.
Shape.
Maybe a broken nail.

A dermatologist sees something else.

Your nails are part of your skin exam, too.

And sometimes, a change in a nail deserves a closer look.

That does not mean every unusual nail is something serious.

And a dark streak does not automatically mean melanoma.

But here’s the important part:

A new or changing dark streak in a fingernail or toenail is something you shouldn’t simply keep covering with polish.

Pay attention to the skin around the nail, too.

Look for changes such as:

A brown or black band that is new or evolving.

Pigment extending onto the surrounding skin.

A nail beginning to lift.

A nail that starts splitting.

A new bump beneath the nail.

The goal is not to diagnose yourself from a photo online.

It’s to know what your normal looks like.

Then notice when something changes.

Because melanoma and other forms of skin cancer don’t always appear as the suspicious mole we’ve been taught to look for.

Melanoma can develop beneath or around a nail.

And skin cancer can occur in places that receive very little sun.

So before your next manicure, take 30 seconds.

Remove the polish.

Look at each fingernail and toenail.

Notice the color.

Look at the nail plate and the surrounding skin.

And if something is new, changing, persistent, or simply doesn’t look right to you, don’t keep covering it and hoping it disappears.

Let a dermatologist look at it.

Medical dermatology isn’t only about acne, rashes and moles.

Your scalp, nails, palms, soles and the spaces you rarely think about are skin, too.

If you’ve noticed a nail change—or you’re overdue for a professional skin cancer screening—this is your reminder to get it checked.

Don’t wait until a small question becomes a bigger one.

It’s Skin Care Awareness Month, and the trends will keep coming, but the fundamentals don’t change: a gentle cleanser, an antioxidant like vitamin C, a retinoid at night and SPF every single day. That’s what decades of dermatology research actually support. ✨
