Facial balancing after weight loss is rarely about adding volume everywhere. It’s about figuring out where structural support has been lost and restoring it strategically.

This is a work in progress.

Weight loss can change more than facial fullness. Loss of support through the midface and lower face can make folds appear deeper, the jawline less defined, and the transition between the cheeks, chin and neck less structured.

For this patient, we’re approaching the face as a whole rather than chasing individual lines.

So far, we’ve treated:

• Cheeks for midface support and contour
• Piriform region to support the area beside the nose and improve the transition into the nasolabial fold
• Chin for projection, proportion and lower-face support
• Jawline to improve definition and the transition into the neck
• Lips to restore subtle shape and volume while keeping them proportional to the rest of the face

The goal isn’t to make her look “filled.” It’s to rebuild support where it makes anatomical sense and gradually restore facial balance after weight loss.

And we’re not finished yet.

Facial balancing is often a process, particularly after significant changes in facial volume. Staging treatment allows us to reassess the face as it changes and decide where additional support will actually improve the result rather than simply adding more product.

Rosacea after one IPL treatment.

Rosacea is a chronic inflammatory condition, but some of what we see clinically is vascular: persistent facial erythema, flushing and superficial dilated blood vessels called telangiectasias.

IPL can specifically target that vascular component.

The light is absorbed by hemoglobin within superficial blood vessels and converted to heat, causing controlled thermal injury to the targeted vessels. Over time, those vessels are cleared, which can reduce both visible telangiectasias and the background redness associated with rosacea.

This is after just one IPL treatment.

You can already see a reduction in diffuse redness across the cheek and a noticeable decrease in the number and prominence of superficial vessels.

But you can also still see redness and visible vessels, and that’s important.

IPL isn’t a one-and-done treatment for rosacea. Clinical studies have demonstrated improvement using a series of treatments, commonly performed approximately 3 to 4 weeks apart. This patient is completing a series of three treatments, with each visit giving us an opportunity to reassess her response and treat the remaining vascular targets.

And we aren’t “curing” her rosacea.

Rosacea is chronic. IPL is treating the vascular manifestations of the condition. Treated vessels can clear, but rosacea can continue to produce new vascular changes over time. That’s why long-term management includes appropriate skincare, daily sun protection, identifying individual triggers and, when indicated, periodic maintenance IPL.

For her, the plan is three IPL treatments followed by maintenance based on how her skin responds over time.

Botox works by temporarily reducing muscle contraction. When the frontalis isn’t contracting as strongly or as frequently, the skin is no longer being repeatedly folded in the same places. That gives those etched-in lines an opportunity to soften.

Both of these photos were taken at rest. The second was taken at her two-week follow-up.

She still has movement. She still has some lines at rest. But the overall smoothness of her forehead is significantly improved, and those deeper horizontal lines have softened without making her forehead look completely immobile.

For many patients, that’s the goal. Less repetitive movement, softer lines at rest, and a forehead that still looks like yours.

What is hyperhidrosis?

Hyperhidrosis is a condition that causes excessive sweating beyond what the body needs for normal temperature regulation. When it affects the underarms, sweating can occur even when someone isn’t hot, exercising, or experiencing another typical trigger.

How do we know exactly where to treat?

We use the Minor starch-iodine test.

Iodine is applied to clean, dry skin and allowed to dry. Cornstarch is then applied over the iodine. When sweat reaches the surface of the skin, it reacts with the iodine and starch, turning areas of active sweating dark purple to nearly black.

That reaction gives us a visual map of where sweating is occurring so we can treat the appropriate area.

For axillary hyperhidrosis, we use 100 units of neuromodulator total, reconstituted with 4 mL of saline, and distribute it between both underarms using small intradermal injections throughout the mapped treatment area.

Neuromodulators reduce sweating by blocking the release of acetylcholine from the sympathetic nerve endings that stimulate the eccrine sweat glands, significantly reducing sweat production in the treated area.

Treatment is $800–$1,000 depending on the neuromodulator used.

You can already see a significant reduction in visible brown pigment and sun damage, with a more even overall skin tone.

IPL uses specific wavelengths of light that are absorbed by chromophores in the skin, including melanin in areas of excess pigmentation and hemoglobin in visible redness and vessels. This allows us to selectively target different components of photodamage without removing or resurfacing the skin.

And while one treatment can make a pretty impressive difference, we don’t consider one treatment a complete correction.

For patients with accumulated sun damage, pigmentation and redness, we typically recommend a series of treatments. Each treatment gives us the opportunity to address what remains and progressively improve overall tone and skin quality while treating conservatively and appropriately for the individual patient.
