Sometimes the Problem in Your Legsβ¦ Isnβt Actually in Your Legs.
When someone has leg swelling, heaviness, pain, varicose veins, skin changes, or venous ulcers, we often start by evaluating the veins in the legs.
But what happens when the leg ultrasound doesnβt fully explain the symptoms?
π Sometimes we need to look higher.
The veins in your legs eventually drain into the larger veins in your pelvis and abdomen. If blood flow is narrowed, compressed, obstructed, or otherwise abnormal higher up, it can affect how efficiently blood drains from the legs.
π©Έ An abdominal/pelvic venous ultrasound may evaluate:
IVC (Inferior Vena Cava)
The major vein responsible for returning blood from the lower body back toward the heart.
Iliac Veins
These large pelvic veins receive blood from the legs. Abnormal flow, narrowing, obstruction, or compression can contribute to lower-extremity venous symptoms.
Venous Flow Patterns
Doppler waveforms provide important clues. Changes in normal respiratory phasicity or significant differences between sides can sometimes suggest a problem farther upstream.
β οΈ When might we want to look higher?
Persistent or unexplained leg swelling, especially when one leg is significantly different from the other; recurrent or difficult-to-heal venous ulcers; recurrent DVT; extensive collateral veins; unusual venous Doppler flow patterns; or symptoms that simply donβt match what weβre seeing in the legs.
π‘ The important part?
A good vascular ultrasound exam isnβt just about finding an abnormality. Itβs about asking:
Does this finding actually explain the patientβs symptoms and if it doesnβt, where else should we look?
Sometimes getting to the root of a lower-extremity venous problem means looking beyond the legs.

Venous insufficiency occurs when the valves inside the leg veins arenβt working properly, allowing blood to flow backward and pool in the legs.
And hereβs something many people donβt realize:
You donβt have to have visible varicose veins to have venous insufficiency.

π HOW WE CHECK FOR VENOUS INSUFFICIENCY
A specialized venous reflux ultrasound evaluates both the anatomy of your veins and how the blood is flowing through them.
We look at:
π Direction of Blood Flow
Is blood moving toward the heart or flowing backward (reflux)?
β±οΈ Reflux Time
How long does abnormal backward flow last?
π Vein Size & Anatomy
We evaluate vein size, anatomy, course, prior treatments, and any absent or abnormal venous segments.
π©Έ Deep & Superficial Venous Systems
We evaluate both venous systems for abnormal blood flow, evidence of prior thrombosis, and post-thrombotic changes that may contribute to chronic venous insufficiency.
π§ Proper Patient Positioning
Venous reflux testing is performed with gravity-dependent positioning to accurately assess valve function.

πJune is Lipedema Awareness Month π
June is dedicated to raising awareness about lipedema, a chronic and often misunderstood disorder that causes abnormal fat accumulation, primarily in the legs, hips, buttocks, and sometimes the arms. Lipedema almost exclusively affects women and is frequently mistaken for obesity, lymphedema, or other causes of leg swelling.
It is estimated that up to 11% of women worldwide may be affected by lipedema, though many remain undiagnosed so the percentage is probably underestimated.
The condition is believed to have a genetic and hormonal component and often develops or worsens during puberty, pregnancy, or menopause.
Common Signs and Symptoms
β’ Symmetrical enlargement of the legs and/or arms
β’ Painful or tender fatty tissue
β’ Easy bruising
β’ Heaviness and fatigue of the limbs
β’ Swelling that may worsen throughout the day
β’ Difficulty losing fat in affected areas despite diet and exercise
β’ Feet and hands are often spared, creating a distinct βcuffβ appearance at the ankles or wrists
Lipedema and Venous Disease
Many patients with lipedema also have underlying chronic venous insufficiency or venous reflux, which can contribute to symptoms such as heaviness, aching, swelling, and discomfort. Because these conditions frequently coexist, identifying and treating any underlying venous disease may help improve symptoms and quality of life.
How Providers and Sonographers Can Help
While there is no single ultrasound test that diagnoses lipedema, healthcare providers and vascular sonographers play an important role in recognizing its signs and ruling out other conditions.
Providers and sonographers can:
β’ Recognize the characteristic distribution of fat and swelling
β’ Obtain a thorough history, including family history and symptom onset
β’ Evaluate for venous insufficiency using venous duplex ultrasound
β’ Assess for lymphedema or other causes of lower extremity swelling
β’ Identify patients whose symptoms seem disproportionate to their weight or lifestyle
β’ Refer patients to specialists familiar with lipedema when appropriate
For many patients, validation is the first step. Acknowledging that their symptoms may be related to a medical condition- not simply weight gain- can lead to earlier diagnosis, better management, and improved quality of life.
Although there is currently no cure, treatment options such as compression therapy, exercise, manual lymphatic drainage, nutritional support, and specialized lipedema reduction surgery may help manage symptoms and slow progression.
Awareness leads to recognition. Recognition leads to diagnosis. Diagnosis leads to better care. π
