Sciatica: The Pain May Be New. The Pattern May Not Be.
When someone comes in with sciatica, one of the first questions they usually ask themselves is, “What did I do?” They think back over the last few days looking for the wrong lift, the awkward step, or the moment they somehow hurt their back. Sometimes there really is an obvious event. But very often my question is different: What have you been doing repeatedly for the last five, ten, or twenty years? Because the day the pain showed up and the day the problem began aren't necessarily the same day.
Take sitting. The human body is remarkably good at adapting to what we repeatedly ask it to do. Spend enough hours sitting and the tissues around the hips can begin adapting to that position. Movement through the hips may become more limited, certain muscles may become more dominant while others contribute less, and the pelvis and low back may begin changing the way they handle movement and load. None of this means that sitting automatically causes sciatica. It means the body may gradually develop a different mechanical strategy for accomplishing the same everyday jobs.
And here's the important part: adaptation isn't necessarily a bad thing. It's one of the reasons we're able to function despite everything we do to ourselves. If the hips aren't moving well, the pelvis can move differently. If the pelvis isn't providing enough movement or stability, the low back can pick up some of the work. The body compensates because its immediate priority is keeping you functional. The problem comes when a temporary workaround becomes the everyday operating system. Eventually those compensations can become expensive, and tissues that have been quietly carrying the extra workload may finally start complaining.
That's why sciatic pain can sometimes seem to come out of nowhere. Yesterday you were fine; today your buttock is burning and pain is running down your leg. But pain is often the notification, not necessarily the beginning of the story. The mechanical environment that contributed to the irritation may have been developing quietly for years. This is also why simply chasing the painful area doesn't always produce lasting results. If we calm the complaint but leave the movement pattern that helped create it untouched, the body may eventually recreate the same problem.
That's where my approach to Medical Massage Therapy becomes a little different. Of course I want to know where you hurt, but I also want to know why your body is asking that area to work the way it is. How are the hips moving? What is the pelvis doing? What is the low back compensating for? What happens when you stand and walk? Sometimes we have to follow the trail backward before the pain starts making sense. So when your sciatica suddenly shows up and you're wondering what you did yesterday, remember: the pain may have started last week, but the pattern may have been building for years.

SCIATICA: What if where it hurts isn't where the problem is?
Sciatica has a way of getting your attention. Burning, aching, numbness, tingling, or pain running through the buttock and down the leg can make the painful area seem like the obvious place to attack. So we rub it, stretch it, heat it, ice it, and try to calm the inflammation. Sometimes that provides some much-needed relief. But when the same pain keeps coming back, I think we need to ask a different question: Why does the body keep recreating it?
The body doesn't move as a collection of separate parts. Your foot affects the way your leg moves. Your leg influences the hip. The hip influences the pelvis, and the pelvis changes what is demanded of the low back. If one part of that chain isn't doing its share, the body doesn't simply stop. It compensates. Another muscle works harder. Another joint changes its movement. Another area tightens to create stability. The body finds a way to keep you moving—even when that solution isn't particularly economical.
Over time, those compensations can become problems of their own. That's why I don't automatically assume the place that hurts is the place where everything began. The painful spot may simply be the guy who finally filed the complaint. If all we do is quiet that complaint without looking at the pattern that created it, we may get temporary relief while leaving the reason for the complaint sitting right where we found it.
That's one of the differences in how I approach Medical Massage Therapy. I'm interested in the pain, but I'm also looking at posture, movement, muscle balance, restrictions, guarding, and the compensations your body has developed along the way. Sometimes the answer is close to where you hurt. Sometimes we have to follow the trail. The objective isn't to promise a magical “fix” for every case of sciatica. It's to give the body better mechanical options so it doesn't have to keep relying on the same expensive pattern.
If you've been treating the same sciatic pain over and over only to have it return, maybe it's time to stop asking only “How do I make this stop hurting?” and start asking “Why does my body keep making this hurt?” That's a very different question—and sometimes it leads us in a very different direction. Vitamin D, magnesium, and vitamin K2 can provide important nutritional support for healthy muscles, bones, circulation, and soft tissue. But nutrients do not work in isolation. They work inside a living, moving body. If that body is spending its days fighting poor posture, restricted movement, chronically guarded muscles, shallow breathing, and layers of compensation, simply supplying the raw materials does not necessarily correct the environment those materials have to work within. Good nutrition supports the terrain. The terrain still has to function.
This is where Medical Massage Therapy becomes important. MMT looks beyond the painful spot and asks why the body is holding itself the way it is. A shortened muscle may be protecting something. A tight shoulder may be compensating for the position of the rib cage. A painful hip may be working overtime because the opposite side isn't carrying its share of the load. When these patterns accumulate, movement becomes harder and more expensive. MMT works to ease those restrictions, reduce unnecessary guarding, improve mobility, and give the body an opportunity to move with less compensation.
That matters because circulation and movement are part of how living tissue is supplied and maintained. Muscles need oxygen, nutrients, fluid exchange, neurological communication, and the ability to contract and release. Vitamin D supports normal muscle and bone function. Magnesium participates in energy production and normal neuromuscular function. Vitamin K2 activates proteins involved in calcium management and normal mineralization. But none of them eliminates a mechanical restriction. If the body is continually fighting itself just to stand, walk, breathe, and work, it is spending resources maintaining the compensation instead of operating efficiently.
Think of it this way: you can deliver all the building materials in the world, but somebody still has to open the road to the construction site. MMT doesn't replace good nutrition, and vitamins don't replace bodywork. They address different sides of the same problem. Nutrition provides materials the body needs. Medical Massage Therapy works with the mechanical environment—muscle, fascia, posture, movement, breathing, and compensation—to reduce unnecessary restrictions and help create better conditions for normal physiology to do what it already knows how to do.
That is the real goal of Body Balancing. We aren't forcing the body to heal, and we aren't pretending one vitamin or one massage fixes everything. We are trying to remove unnecessary obstacles and give the body better conditions in which to function. Support the chemistry with Vitamin D, magnesium, and K2. Support the structure with movement, posture, circulation, and Medical Massage Therapy. When structure and chemistry stop fighting each other, the body has a much better opportunity to get back to what it does best: adapt, repair, move, and get you back to life.
