You may notice a few new forms before your next appointment.

I’ve made a few updates behind the scenes at XO Glam Beauty to help keep appointments, client information, and treatment records a little more organized.
Because of these updates, you may receive a request from Square to complete or update forms before your next appointment, even if you’ve been coming to me for a while.

Depending on your appointment, this may include:
- An updated XO Glam Beauty Client Agreement
- Credit Card Authorization
- A service-specific intake or consent form based on what you’re booked for

Some forms are completed once, while others may need to be updated periodically or when you book a different type of service.

If Square sends you a form, please complete it before your appointment. This gives me time to review anything important beforehand and helps us spend more of your appointment time focused on you instead of paperwork.

And don’t worry, receiving a new form doesn’t mean anything is wrong with your appointment or account. I’m simply updating my systems and making sure I have the information I need to provide your services safely and efficiently.
