Lifestar Case Management Services, Inc. has been active since the beginning of 2012. We are an agency approved and audited annually by Florida Medicaid. The purpose of the program of Mental Health Targeted Case Management Services is to instruct step by step our clients to gain access to financial benefits, necessary medical services, transportation, housing, medical coordination, social, educational, personal care and other identified needs. Linking them with the financial, medical, health care and other institutions of community support, and other services. On March 2013 We beginning to expand, and we opened the New Branch Office in Key West Florida, we are covering the areas of Miami Dade and Monroe County.
