I get asked about insurance every single day. Here is the honest answer.

Insurance pays for what they call "medically necessary" care.
Their definition is narrow: you have to already be sick.
Sub-optimal but not yet diagnosable doesn't count.

The work I do — longer visits, specialty testing, lifestyle work, hormone optimization — is by definition not what insurance is built to reimburse. So the majority of true functional medicine clinics, including mine, are cash-pay.

It's not a billing failure. It's a structural choice.
I don't build my care around what insurance covers.
I build it around what actually helps you.

This is the conversation I have with patients every single week.

The 2002 Women's Health Initiative study scared a generation of women off HRT. The interpretation of that study has been heavily revised since. The data has changed. Most doctors haven't caught up.

Estrogen protects the brain. In both sexes. Men keep aromatizing testosterone into small amounts of estrogen throughout life — and that's part of why their Alzheimer's rates are lower. Women lose their estrogen at menopause, and most have been refused the hormone that would have continued protecting their brain.

I'm not saying HRT is for everyone. I'm saying the reflexive fear of HRT is doing women real harm — and if your doctor refuses to discuss it without showing you current data, find another doctor.

If your labs are "normal" and you still feel terrible — you might not be crazy. You might just be sub-optimal. That's a fixable problem, but conventional medicine isn't built to find it.

Conventional reference ranges come from the general population. Most of whom are not well. So "normal" means you don't have a diagnosable disease yet. It does not mean optimal.

The single most important sentence I say in every Discovery Visit.

If you've been told you're "fine" but you don't feel fine — that's the gap between normal and optimal. And it's where my work lives.

Conventional medicine asks if something is abnormal.
Functional medicine asks if it's optimal.
They are not the same question.
