The conversation has moved beyond debating glucagon-like peptide-1 (GLP-1) medications. The new question is: What tissue comes off: and what stays?

A 2026 meta-analysis of 20 randomized trials found lean mass accounted for about 25–39% of weight lost: semaglutide 35.2%; tirzepatide 25.4%; liraglutide 26.8%; lifestyle 26.2%; lifestyle plus resistance training 17.5% (Eisa & Barood, 2026). Exercise plus liraglutide preserved lean mass better than liraglutide alone in a randomized trial (Lundgren et al., 2021). GLP-1 therapy does not automatically mean muscle wasting; scale weight alone is simply incomplete.

A self-reported analysis of 410,000+ Reddit posts from 67,000+ users found 43.5% reported a side effect; fatigue appeared in 16.7%, and reproductive or menstrual symptoms in about 4%. It is observational and hypothesis-generating: not proof of causation: https://www.sciencedaily.com/releases/2026/09/260912222856.htm

The precision protocol: measure body composition, not just weight, with a DEXA scan where available; perform progressive resistance training; consume adequate protein; and coordinate medication, nutrition, and bloodwork with the prescribing clinician.

A static list of bacterial names is not the full microbiome story. 🧬

Advanced testing can examine what microbes are doing and which biological pathways are active, not only which organisms are present. RNA-based metatranscriptomic analysis is one approach used to study microbial activity and functional signals. We use for gut health testing.

But data is context, not a diagnosis, and it is not a guaranteed explanation for symptoms or a promise of improved gut health.

A useful interpretation considers the complete picture:

• Nutrition and fiber intake
• Medication history
• Gastrointestinal tolerance
• Sleep quality
• Training load and recovery
• Relevant clinical data and biomarkers

The goal is not to chase a perfect bacterial score. It is to build a practical, individualized roadmap from better information.

POSH coaching helps you interpret health data and turn insights into sustainable nutrition, training, and behavior strategies. We provide practical support, not medical treatment.

Your aging muscle may be changing its identity because its mitochondrial membrane is losing a critical lipid. 🧬

Cardiolipin sits in the inner mitochondrial membrane and supports energy production. A new Nature Aging study found it declines with age in mouse and human muscle.

That decline triggered a glycolytic-to-oxidative fiber switch through ERRγ: a reactive-oxygen-species-sensitive, potentially protective response, not simply damage.

The paradox: aged muscle can look more oxidative on paper while performing worse. More mitochondria-related machinery does not equal better muscle function.

In mice, restoring cardiolipin synthesis improved muscle outcomes and rescued premature mortality. This was mouse work. No human therapy exists for this pathway, and no supplement has been proven to reproduce it.

For adults over 40, the levers we can pull remain practical: aerobic base work, progressive resistance training, adequate protein, sleep, and recovery. Mitochondrial-support supplements are adjuncts guided by measurement, never substitutes.

Mechanism research tells us where to look, not what to buy.

The scale is not the whole story. ⚖️

When weight comes off with a GLP-1 medication, the goal should be to lose excess fat while protecting muscle, strength, and function.

A new Northwestern University study offers an intriguing early signal: in mice, an experimental RNA therapy paired with semaglutide reduced lean-mass loss to about 5.5%, compared with roughly 10% with semaglutide alone. The combination also improved metabolic outcomes and encouraged fat to burn more energy.

Important: this was mouse research. The RNA therapy is experimental, not available as a treatment, and these findings do not automatically translate to humans. Human evidence shows that GLP-1 treatment can reduce lean mass along with fat. Evidence on bone mineral density and fracture risk remains limited, with no clear overall signal.

The practical takeaway is simple: if you are using a GLP-1 under medical supervision, track more than body weight.

✅ Prioritize resistance training.
✅ Consume adequate protein.
✅ Monitor strength and body composition.

NMN, NR, and nicotinamide are different NAD+ precursor compounds, not interchangeable shortcuts to longevity. 🧬

Human studies suggest that NR and NMN can raise circulating NAD+ metabolites. Nicotinamide follows a different metabolic route and may produce different timing and downstream effects.

But a short-term biomarker change is not proof of improved energy, performance, healthspan, or lifespan.

A precision review asks:
• What is the exact ingredient identity?
• What dose and duration were studied?
• What is the total stack exposure, including other supplements and fortified foods?
• Which bloodwork, medications, and health conditions could change the risk-benefit equation?
• What outcome is actually being monitored: NAD+ metabolites, glucose control, exercise capacity, symptoms, or a validated clinical endpoint?

The foundation remains non-negotiable: progressive resistance training, cardiovascular fitness, sufficient protein, fiber-rich foods, quality sleep, and appropriate medical care. Upgrade the protocol only after the fundamentals are engineered.

Is the classic 20–30 g protein-per-meal rule still precise after 40? 🧬

Two 2026 trials suggest the answer may depend on the individual.

In older males, 20 g of a plant-derived protein blend, with or without added leucine or whey, did not increase muscle protein synthesis above basal rates.

In a separate randomized crossover trial, 40 g of whey increased muscle protein turnover in older adults. Adding 3 g of HMB enhanced the response in older women, but not older men. That finding was sex-specific.

The precision-first takeaway:

• Protein source and total dose matter.
• A generic 20–30 g target may undershoot for some adults.
• These were acute, single-meal laboratory outcomes, not proof of long-term muscle or strength gains.
• MPS is a mechanism marker, not a guarantee of muscle gain.

Your optimal target depends on body size, training status, kidney health, medications, and medical history. Anyone with kidney disease should speak with their clinician before increasing protein.

Do not self-diagnose from one study. Build a coached, tracked protein strategy around your data, training, and clinical context.

The FDA vote is not a green light. 🧬

In July 2026, an FDA advisory panel voted to recommend easing restrictions on six peptides: BPC-157, MOTS-c, KPV, TB-500, Epitalon, and Semax.

Important distinction: this was a non-binding recommendation, not law, not FDA approval, and not authorization for general use. Finalization may not occur until roughly 2027–2028.

KPV, Epitalon, and Semax are receiving increased attention, but evidence and regulatory status vary by compound and jurisdiction. Large, well-controlled human data remain limited.

That is why clinical oversight matters more than ever.

In collaboration with GameDay Men's Health Westport, Posh Fitness supports a precision-first process that may include:

• Physician evaluation
• Medical-history and medication review
• Labs when clinically indicated
• Eligibility screening
• Informed consent
• Ongoing monitoring

No gray-market sourcing. No “research use only” products. No online purchasing. No self-prescribed dosing.

Your biology is not a DIY experiment. Ask better questions. Use qualified clinicians. Demand accountability.

Your DNA is largely fixed. Your health data is not. 🧬

That distinction matters.

A DNA report is a one-time context layer. It can help identify tendencies that may influence how you respond to training, nutrition, recovery, or certain nutrients. But it is not a permanent verdict, and it should not become a permanent plan.

The data that moves deserves regular attention:

• Blood pressure: track trends regularly, especially when lifestyle or medication changes.
• Bloodwork: often reassessed every 3–12 months, depending on the marker, your history, and clinician guidance.
• Body composition and fitness metrics: review roughly every 8–12 weeks when actively changing your program.
• Sleep and recovery: monitor continuously, then review weekly for patterns.

Repeat DNA testing is usually unnecessary because your inherited sequence does not meaningfully change. What changes is your environment, behavior, training load, sleep, body composition, and biomarkers.

Precision coaching connects the layers. For example: a genetic tendency related to lipid handling, current ApoB and LDL-C results, nutrition history, and training consistency may point toward a specific priority, such as improving aerobic capacity, increasing resistance training, or refining dietary fat quality. The priority can change when the data changes.

A coach can help organize trends and build practical habits. Questions involving diagnosis, medications, abnormal results, or treatment belong with a qualified clinician.

Your DNA informs the roadmap. Your current data determines the next turn.

A DNA result should create a better conversation, not a fixed identity.

The androgen receptor (AR) gene includes a CAG-repeat region studied in relation to androgen signaling and muscle-related traits. The research is not a prescription, and a genetic result cannot tell someone exactly how they will respond to resistance training.

Why is it still useful? Coaching should account for the full picture:

• Training history and current strength
• Movement quality and injury history
• Recovery, sleep, and stress
• Hormone and metabolic lab context
• Medications and health conditions
• Goals, preferences, and consistency

A genetic association is probabilistic. It does not replace progressive resistance training, adequate protein, recovery, or medical evaluation when symptoms are present.

At POSH Fitness, genetic information is one data point in a broader health strategy: translated into measurable habits, training priorities, and follow-up metrics.

Your biology provides context. Your plan still has to work in real life.

“Zombie cells” have a measuring stick now. 🧬

MIT researchers recently reported RamanOmics in Nature Aging: an AI-assisted method that combines label-free Raman imaging with spatial transcriptomics to identify senescent cells inside tissue.

The system detected a conserved lipid-associated Raman signature around 1,131–1,135 cm⁻¹ and used it to recognize p21-positive senescent cells in mouse lung and skin tissue.

Why does this matter?

The bottleneck in senescence research has not been a shortage of senolytic candidates. It has been detection.

You cannot manage what you cannot reliably identify.

This is precisely why a test-before-you-treat posture matters. Before pursuing an intervention, establish what is actually measurable, track the trend, and distinguish biological signal from biological noise.

RamanOmics is promising, but it is still preclinical methodology research. It is not a clinical test you can order... yet.

Until cellular-senescence measurement matures, track the fundamentals that already matter: strength, aerobic capacity, body composition, and bloodwork trends.
