What is BFR (Blood Flow Restriction) Training?
At our clinic, we use BFR SmartCuffs as part of select rehab programs to help patients build strength and recover faster—often with much lighter weights than traditional training.
So how does it work?
BFR uses a specialized cuff placed on the arm or leg to gently and safely reduce blood flow out of the working muscle during exercise (while still allowing blood in). This creates a unique training environment where your muscles “think” they’re working much harder than they actually are.
That means you can get a high-intensity training response using low loads.
How we use it in physical therapy:
We incorporate BFR SmartCuffs during:
• Post-op rehab (ACL, shoulder, meniscus, etc.)
• Tendon injuries (tendinopathy)
• Early strengthening phases when heavy loading isn’t safe yet
• Patients with pain or joint irritation that limits lifting
Instead of heavy weights, patients perform controlled, low-load exercises while wearing the cuffs under clinician supervision.
⚡ Why it can be so beneficial:
✔ Builds strength when heavy weights aren’t tolerated
✔ Helps reduce muscle atrophy after injury or surgery
✔ Can accelerate early-stage rehab progress
✔ Improves muscle endurance and activation
✔ Joint-friendly alternative to traditional strengthening
It’s one of the tools we use to bridge the gap between “I can’t lift heavy yet” and “I’m ready to train normally again.” Not every patient is a candidate, but when appropriate, BFR can be a game-changer in rehab progression.
