The LIPLEG trial, a landmark study and the first large, multi-center, randomized controlled trial to directly compare liposuction with conservative therapy for Lipedema, was published this week in The Lancet (Podda et al., 2026). This paper is the strongest evidence to date that Lipedema pain is treatable and that liposuction can help many patients.

The trial found that liposuction (often combined with conservative therapy) produced significantly greater reductions in pain than conservative therapy alone. After 12 months, 68% of the liposuction group had a meaningful reduction in leg pain, compared with just 8% of the conservative therapy group, and this result was consistent across all three Lipedema stages. The participants who received liposuction had 26.2 times the odds (an “odds ratio” of 26.2) of reporting a two-point reduction in leg pain on a ten-point scale. Put more simply, the participants in the liposuction group were roughly nine times as likely to achieve that pain reduction (68% vs. 8%). This figure may even be a conservative estimate; in 41 of the 278 surgical cases, researchers were unable to complete the 12-month follow-up assessment and counted these as treatment failures, though presumably some participants had positive outcomes.

The liposuction group also reported significant improvements in quality of life, physical functioning, and tendency to bruise. The improvements in mental health and in some quality-of-life measurements (like social relationships) were smaller, and in a few cases not statistically significant, especially in the earliest and most advanced Lipedema stages.

The trade-off for these results is safety. As might be expected with an invasive procedure, adverse events were more common after liposuction compared to the group treated with only conservative therapy (47% vs. 21%), including a higher rate of serious events (7.5% vs. 3.4%). Most adverse events were reported to be mild to moderate and included things like surgical site infections, drops in blood count, and fluid buildup under the skin. None of the adverse events were unexpected or life-threatening.

In this study, 410 women with stage I, II, or III Lipedema across 11 German medical centers were randomly assigned to either liposuction (with optional concurrent conservative therapy) or conservative therapy alone. The LIPLEG researchers took several steps to reduce bias: the people assessing each patient's outcomes did not know which treatment they had received, and patients were asked not to reveal their treatment. The researchers acknowledge that visible changes in leg shape after surgery could still have revealed the treatment in some cases.

Some considerations:

- These results reflect surgery performed by experienced Lipedema surgeons, under strict safety limits, after months of conservative therapy first. Anyone considering liposuction should seek out surgeons with specific Lipedema experience and discuss both the potential benefits and the elevated risk of complications. Surgery is invasive and may not be the answer for many patients.

- Important questions remain open. These are 12-month results; the trial's 36-month follow-up is ongoing and will tell us whether pain relief lasts and whether symptoms recur. The study was conducted entirely in Germany, and it excluded patients over 120 kg and those with significant arm involvement, so results may not apply equally to everyone.

These results are already having real-world impact. The LIPLEG trial has changed policy in Germany, where statutory health insurance now covers liposuction for eligible Lipedema patients at all stages. We see this as a template for the evidence-based coverage conversations that need to happen urgently in the US and elsewhere.

Worked on a post-op lipedema client today who had liposuction with a surgeon who specializes in lipedema liposuction in California. He provided the client with this foam (the one sided version) and the results are already mind blowing after less than two weeks.

Today is **World Lymphedema Day** — a day to raise awareness, show support, and advocate for those living with lymphedema.

Lymphedema is a chronic condition that causes swelling, often in the arms or legs, due to a buildup of lymph fluid. It can result from cancer treatments, surgeries, or primary conditions affecting the lymphatic system. While lymphedema is common, it is often misunderstood and underdiagnosed.
This day is about breaking the silence and promoting better understanding of the challenges patients face every day. Early diagnosis, proper treatment, and ongoing care can make a huge difference in quality of life.

If you or a loved one is living with lymphedema, know that you are not alone. Let’s continue to spread awareness, advocate for better care options, and support the lymphedema community.

Ways to show your support:
- Wear blue or light green 🎗️
- Share your story or a loved one’s journey on social media
- Use hashtags like
- Educate yourself and others about the condition

Together, we can shine a light on lymphedema and advocate for better care worldwide.
