✨ Spotting or unscheduled bleeding after starting HRT is very common, especially in the first 3–6 months. It often settles as the endometrium adapts and does not usually mean something serious, but the expected pattern and when to investigate depend on the regimen and how long you’ve been on it.
What’s expected by regimen
🔹Sequential (cyclical) HRT: A monthly withdrawal bleed toward the end of the progestogen phase is normal. Extra spotting or irregular bleeding outside that window is unscheduled.
🔹Continuous combined HRT (daily estrogen + progestogen, typical after 12 months of amenorrhea): The goal is amenorrhea. Light spotting or irregular bleeding is common while the lining stabilizes and frequently improves by 3–6 months. After that, bleeding is unscheduled. 
Unscheduled bleeding occurs in roughly 40% of users early on. It is more common if HRT is started in perimenopause (when residual follicular activity is still present) or after an estrogen dose increase. 
Why it happens?
Estrogen stimulates the endometrium; progestogen keeps it thin and stable. When HRT is started or the dose/route is changed, receptors “wake up” and the lining can shed irregularly until it down-regulates. Inadequate progestogen relative to the estrogen dose, missed doses, poor patch/gel absorption, or concurrent medications can worsen it.
Most early bleeding is the lining settling, not cancer. The risk of endometrial cancer on appropriately opposed HRT at standard doses is low, but persistent or late-onset bleeding still needs a systematic check so treatable causes (polyps, hyperplasia, inadequate opposition) are not missed. A clinician familiar with the exact products, doses, and the patient’s menopausal stage can usually sort this out with a targeted history and, when needed, imaging.
