☑️Very proud to share our new study, now published in the European Journal of Radiology.
☑️In this single-center study of 247 patients, ✔️based entirely on data from our own institution, we found that os acromiale is associated with higher critical shoulder angle and acromial index, a markedly higher prevalence of AC step-off deformity and glenohumeral osteoarthritis, and a selective pattern of rotator cuff involvement, particularly affecting the infraspinatus and subscapularis, while supraspinatus tear rates were not significantly different.
☑️To the best of our knowledge, this is the first study to bring os acromiale, scapular morphometry, rotator cuff pathology, joint degeneration, and radiograph–MRI agreement together within the same cohort, suggesting that os acromiale may represent more than simply an incidental anatomical variant.
☑️Especially proud of our team and of seeing a question arising from our own daily practice develop into a study we could build together.

☑️Our new study is now published in Skeletal Radiology! 🎉
☑️We add another structure to the posterior stabilizing anatomy of the lateral meniscus: ✔️the semimembranosus–lateral meniscus (SM–LM) attachment.
☑️Previously described anatomically, we characterized this attachment on routine in vivo MRI, determined its prevalence, and explored its relationship with the Humphrey and Wrisberg ligaments and the oblique popliteal ligament. Interestingly, the SM–LM attachment showed an inverse association with the OPL, suggesting a more variable organization of the posterior stabilizing network than previously appreciated.

☑️ I am delighted to share that our new study has been published in Skeletal Radiology:
“Performance of multimodal large language models versus clinicians for radiographic knee osteoarthritis grading: A multiobserver study”
☑️In this retrospective, single-centre study of 348 weight-bearing knee radiographs, we compared ChatGPT-4o and ChatGPT-5.0 with clinicians of different experience levels in Kellgren–Lawrence grading of knee osteoarthritis.
☑️Our findings indicate that current multimodal LLMs are not suitable as standalone radiographic interpreters. They may be regarded as promising adjuncts, but require rigorous optimization, validation and supervised integration before routine clinical use.
☑️The study was designed under the leadership of dear Irmak Akdoğan who ran the project with great dedication, and Efe Kemal Akdoğan contributed valuable input at every stage. Our residents Sılanaz Kutlu Sılanaz and Fatih Tümer Fatih worked with remarkable commitment, while I contributed and provided guidance whenever needed. At times they guided me, and at times I guided them, resulting in a truly rewarding team effort built on mutual learning.

☑️A 27-year-old patient presented with pain and swelling of the left 4th metacarpophalangeal joint.
☑️Plain radiography demonstrated subtle/diffuse calcification along the course of the radial collateral ligament of the 4th MCP joint.
☑️MRI showed marked periarticular edema and joint effusion, along with tiny T2-hyperintense foci in the adjacent bones, likely related to disuse osteopenia.
☑️The imaging findings are consistent with acute calcific periarthritis secondary to hydroxyapatite crystal deposition disease.
☑️This case highlights the importance of carefully looking for subtle periarticular calcifications on plain radiographs, especially when evaluating pain and swelling in small joints.
☑️☑️Teaching point:
In small joints, the combination of periarticular calcification + marked surrounding edema should raise the possibility of hydroxyapatite deposition disease.

☑️27 yaşında hastada sol 4. metakarpofalangeal eklemde ağrı ve şişlik mevcuttu.
☑️Direkt grafide 4. MKF eklem radial kollateral bağ trasesinde ince/diffüz kalsifikasyon izlendi.
☑️MRG’de eklem çevresinde belirgin ödem ve efüzyon; komşu kemiklerde ise kullanmamaya bağlı osteopeni ile ilişkili milimetrik T2 hiperintens odaklar dikkati çekti.
☑️Görüntüleme bulguları, hidroksiapatit kristal depo birikim hastalığına bağlı akut kalsifik periartrit ile uyumludur.
☑️Bu olgu, özellikle küçük eklemlerde ağrı ve şişlik değerlendirilirken direkt grafideki ince periartiküler kalsifikasyonların dikkatle aranmasının önemini hatırlatıyor.
☑️☑️Öğretici nokta:
Küçük eklemlerde periartiküler kalsifikasyon + belirgin çevresel ödem birlikteliği, hidroksiapatit depo hastalığı açısından akılda tutulmalıdır.

☑️Laboratuvarda lökositoz, yüksek sedimentasyon ve CRP mevcuttu.
☑️Sakroiliak MRG’de eklem komşuluğundaki kemik ve yumuşak dokuda yoğun ödem benzeri sinyal değişiklikleri , öncelikle septik sakroiliit. Kültürde S. aureus üredi; tedavi hızla başlandı.
☑️Öğretici Nokta: Septik artrit kas-iskelet sisteminin acillerindendir. ✔️Tanı klinik + mikroorganizmanın üretilmesiyle konur; ancak görüntüleme erken tanı, yaygınlık ve komplikasyonların (implant gevşemesi, abse) gösterilmesinde kritiktir. İmplant çevresindeki radyolusensi her zaman “sadece mekanik gevşeme” değildir, enfeksiyon ekarte edilmelidir.
🏴󠁧󠁢󠁥󠁮󠁧󠁿Fever + hip pain + a loosening screw: don’t miss it!
☑️A 65-year-old woman with prior lumbosacral instrumentation presented with fever and right hip pain.
☑️The AP radiograph showed a radiolucent line around the fixation material at the inferior right sacrum (loosening), plus irregular widening and erosions of the right SI joint.
☑️Labs revealed leukocytosis with elevated ESR and CRP.
Sacroiliac MRI demonstrated intense edema-like signal in the periarticular bone and soft tissues, favoring septic sacroiliitis. ✔️Cultures grew S. aureus, and treatment was started promptly.
☑️Teaching point: Septic arthritis is a musculoskeletal emergency. Diagnosis rests on the clinical picture plus isolating the organism, but imaging is essential for early detection, mapping extent, and revealing complications (implant loosening, abscess). Peri-implant lucency is not always “just mechanical loosening”, always exclude infection.

☑️Bone erosion is only half the story. The other half is the silent dismantling of the stabilizers.
☑️In chronic inflammatory wrist disease, an absent TFCC together with scapholunate injury may be an important sign of evolving DRUJ and carpal instability.

25 Haziran’da TRD Kas-İskelet Radyolojisi Eğitim ve Bilimsel Araştırma Grubu toplantısında birlikte olacağız.

İnflamasyonu göstermek artık tek başına yeterli değil. Sakroiliak eklem görüntülemesinde yapısal lezyonların doğru değerlendirilmesi giderek daha fazla önem kazanıyor. CT-benzeri MR teknikleri sayesinde MR, tek duraklı görüntüleme yöntemi olma hedefine her geçen gün biraz daha yaklaşıyor.

Detecting inflammation alone is no longer enough. Accurate assessment of structural lesions has become increasingly important in sacroiliac joint imaging. With the advent of CT-like MRI techniques, MRI is moving closer than ever to becoming a true one-stop imaging modality.
