🦷 The Hidden Anatomy That Determines the Perfect Maxillary Anterior Arch

Did you know that the ideal position of maxillary anterior teeth can be estimated even when all natural teeth are missing?

Long before digital dentistry, prosthodontists relied on stable anatomical landmarks to reconstruct a natural-looking dental arch.

Step 1️⃣ Draw the Midline

Use:

* Anterior: the labial frenum
* Posterior: the fovea palatinae

If the fovea palatinae cannot be identified (common in elderly patients), use the median palatal suture instead.

Step 2️⃣ Locate the X Point

Identify the region corresponding to the second premolars.

Since the palatal rugae normally do not extend beyond the second premolars, this landmark is easy to estimate on an edentulous cast.

Draw a line connecting points located approximately half a premolar cusp width anterior to the distal surfaces of the second premolars.

Where this line intersects the midline is called the X Point.

Step 3️⃣ Estimate the Canines

✅ Anatomically, the terminal end of the first palatal ruga generally points toward the distal aspect of the canine.

Connecting this landmark to the X Point allows the ideal curvature of the anterior arch to be reconstructed.

✅ Using these anatomical references, the center and curvature of the maxillary anterior dental arch can be calculated—even in completely edentulous patients.

✅ This method was demonstrated by Kakutaro Aiba in 1958, and remains a fascinating example of anatomy guiding prosthodontic treatment.
