What can we see here?
A combination of closed comedonal congestion, small inflammatory lesions, post-inflammatory erythema and areas of uneven texture. Different lesion types and residual changes can exist at the same time, which is why acne treatment needs to address both active acne and what it leaves behind, rather than treating every bump the same way.

What comes out during an extraction?
This is a build-up of compacted sebum, keratin and cellular debris that has been manually cleared from congested follicles. When this material becomes trapped within the follicle, it can contribute to blackheads, closed comedones and persistent congestion, which is why some blockages benefit from professional extraction rather than skincare alone.

A closer look at comedonal congestion.
we can see multiple open comedones, where a build-up of sebum and keratin within the follicle is exposed at the surface and oxidises, creating the characteristic dark appearance. When congestion becomes compacted like this, targeted extractions can be used to physically clear suitable follicles as part of a broader treatment plan aimed at reducing ongoing congestion.

Extractions are not about “popping” every breakout. They’re a controlled clinical technique used to clear suitable blocked follicles, including blackheads, closed comedones, milia and persistent congestion.

Not every lesion should be extracted, correct assessment and technique are important to minimise unnecessary inflammation, marking and trauma to the skin.

Progress through a multi-modality treatment approach. We’ve combined microneedling to address texture and support dermal remodelling with Melano Clear IPL to selectively target areas of excess pigmentation. Solar lentigines are a visible record of cumulative UV exposure.
Commonly referred to as sun or age spots, they develop when repeated UV exposure causes localised increases in melanin production, creating flat, well-defined areas of pigmentation. Unlike melasma, solar lentigines are typically discrete lesions rather than diffuse patches and become increasingly common with age and ongoing sun exposure.

Melasma has distinct characteristics.

It typically presents as flat, irregular patches of brown to grey-brown pigmentation, most commonly appearing symmetrically across the cheeks, forehead, upper lip, nose and jawline.

Unlike an isolated sunspot, melasma often forms larger patches with less defined borders and can appear almost “cloud-like” across the skin. The depth and colour can also vary depending on whether pigment is predominantly epidermal, dermal or mixed.

A key characteristic of melasma is its tendency to fluctuate. Pigmentation can become more visible following UV exposure, heat, hormonal changes and inflammation, then appear lighter at other times.
