Here's my take about Knee pain, when climbing stairs usually comes down to high mechanical stress, poor tracking of the kneecap, or inflammation in the joint structures.
Here are the three primary reasons this happens:
1. Patellofemoral Pain Syndrome ("Runner's Knee")
When you step up, your leg presses the back of your kneecap (**patella**) against the thigh bone (**femur**). If the kneecap doesn't slide smoothly along its groove—often due to tight quads, weak glutes, or altered leg alignment—it creates uneven friction. This increased contact pressure is why you feel a dull, aching pain right behind or around the kneecap when loading the joint.
2. Tendonitis or Tendinopathy.
The patellar tendon (connect
ing your kneecap to your shinbone) and the quadriceps tendon (connecting your thigh muscle to your kneecap) act as the primary brakes and springs when navigating steps. Overuse, sudden increases in activity, or poor movement mechanics can cause micro-tears and inflammation in these tendons. Pushing through a step puts high tensile strain on them, causing sharp or localized pain just above or below the kneecap.
3. Cartilage Wear and Joint Stress (Osteoarthritis)
Climbing stairs generates force across the knee joint equal to roughly 3 to 4 times your body weight. If the protective cartilage cushioning the joint has thinned over time, that massive force transfers directly onto the underlying bone and irritated joint linings. This often presents as a deep ache, stiffness, or a grinding sensation (crepitus) during step-ups.

Addressing this usually involves a mix of temporary load management (taking steps one at a time or relying on handrails) and strengthening the supporting muscles—especially the gluteus medius and quadriceps—to improve how the joint handles stress.
