Skincare, treatment or surgery? 👀 How do you know what your eyes actually need?

Dryness and early fine lines may need targeted skincare. Crepey skin may benefit from improving skin quality and, for selected patients, an in-clinic treatment. Hollowing needs careful anatomical assessment.
And when we’re dealing with structural concerns such as persistent lower-eye bags or significant excess upper-lid skin, there comes a point where skincare and non-surgical treatments simply cannot address the underlying anatomy. Surgery may then be the more appropriate option.
This is why I always say: don’t choose the treatment first. Diagnose the problem first.

I can‘t address more enough about stop rubbing and pulling at yo ureyes to remove makeup.
The skin around your eyes is delicate, so your cleansing technique matters just as much as the products you apply afterwards.
My approach is simple: apply a targeted eye cleanse*, gently massage a few seconds to help loosen your makeup, then gently wipe away with cotton pad or rinse off directly.
Your evening eye routine should start with how you cleanse.

Your skincare isn’t working or have you simply not given it enough time?
One of the biggest mistakes I see is constantly changing products because we don’t see an immediate transformation.
Instead, try 3-Month Skincare Test:
➡️ Choose one main concern, take a baseline photo in consistent lighting, follow a targeted routine, and give appropriate products time to work, provided your skin is tolerating them well.
Then reassess.
And if you’ve been genuinely consistent but you’re still not seeing the change you expected, don’t automatically reach for something stronger.
Ask yourself: “Am I actually treating the right problem?”
Pigmentation, dehydration, fine lines, volume loss, skin laxity and structural under-eye bags all require very different approaches and skincare cannot correct every concern.
Sometimes the missing ingredient isn’t another serum. It’s the right diagnosis. 👁️✨

1️⃣ I apply SPF all the way to my eyelids, neck and hands. ☀️
2️⃣ I never went to bed with my makeup on. 🫧
Gentle, thorough cleansing—especially around the delicate eye area—has always been non-negotiable for me.
3️⃣ I use antioxidants consistently to my neck and décolletage too.
4️⃣ Start retinols / peptides slowly rather than waiting until you have visible wrinkles.
5️⃣ Give every product a proper trial and stop constantly switching.
If I had £500 to spend on healthy ageing, I wouldn’t spend it all on treatments.

Not every line under your eyes needs to be treated.
When you smile, the muscles around your eyes move and the skin naturally folds. These dynamic lines are normal anatomy and trying to erase every one of them isn’t my idea of healthy ageing.
What I pay more attention to are fine lines that remain at rest, particularly when accompanied by dryness, crepey texture or changes in elasticity.
That’s when I start thinking about skin quality: hydration and barrier support first, followed by appropriate targeted ingredients, such as peptides including Acetyl Hexapeptide-8.
As an oculoplastic surgeon who created her own periorbital skincare range, I think it’s important to be transparent about that.
Good skincare shouldn’t promise miracles. It should do its job well, for the right indication.
If puffiness is related to dehydration or temporary fluid retention, targeted eye skincare can help support hydration and improve the appearance of the skin, while consistent red light LED therapy may help support overall skin quality over time.
But if what you’re seeing is pr*****ed fat or significant excess skin, no serum, cream or eye mask can physically remove it.
That’s where an accurate diagnosis matters. Sometimes skincare is appropriate. Sometimes an in-clinic treatment may help. And sometimeslower eyelid surgery is the more appropriate conversation.
As we age, our priorities naturally shift.
20s ➡️ PREVENT
30s ➡️ PRESERVE
40s ➡️ STIMULATE
50s+ ➡️ SUPPORT
But this isn’t about suddenly doing more as you get older. It’s about understanding what your skin needs now and adapting accordingly.
Your age is a guide. Your skin quality, lifestyle, anatomy and individual concerns should always determine your routine.

Your best skin starts weeks before the event not the night before.
Whether it’s a wedding, holiday or special occasion, I always recommend planning ahead. Start by optimising your skincare, then as the date gets closer, focus on consistency, cleanse, hydration and protecting your skin barrier.
And Dr Sabrina’s Rule: the week before your wedding is NOT the time to try that viral TikTok skincare routine. ❌
Great event skin is usually the result of weeks of consistency, not a last-minute miracle.
