Walking & Night Pain for TKR TOTAL KNEE REPLACEMENT
š¬ Patient Question:
"What distance do you walk per day, or how long is okay to stand without needing to sit down? "
šæ Physiotherapist Response:
The pain you get at night is the sign that the walk was too far or too long. While the tissue is healing (up to 6 weeks), gradually increase your walking. You only know if you have done too much if you end up having pain at night. Pain at night is a sign of overdoing or pushing a healing tissue too far. After 6 weeks, you should be able to increase your walking more easily, but until then it is important to respect the tissue healing.
Hope this helps! š

Understanding Your Post-Op Knee & That Pinching Pain
It is completely understandable to feel concerned when a standard exercise suddenly brings on sharp, pinching discomfort.
At 8 days post-op, your body is in the acute healing phase. Let's break down why that pinching feeling is happening behind your kneecap during leg lifts, whether you should push through it, and what steps you can take right now to protect your new joint while keeping your recovery moving forward.
Why You Shouldn't "Grit and Bear It"
When you experience a sharp pinch or cramp-like sensation during early rehab, your nervous system is sending a clear protective signal. Pushing through this type of pain is counterproductive for several key reasons:
* Triggering Protective Guarding: If a muscle or tendon is placed under excessive tension and forced to work through sharp pain, the nervous system initiates a protective reflex. It will actively tighten and cramp the surrounding muscles to restrict movement and prevent perceived damage.
* Addressing Pre-Existing Patterns: Surgery replaces the joint surfaces, but it does not instantly erase years of movement compensations, muscle imbalances, or tight fascia that developed while your knee was deteriorating pre-surgery.
* Adapting to New Alignment: Your new knee component may have a slightly different mechanical alignment than your original bone-on-bone joint.
The muscles surrounding the kneeāparticularly the quadriceps, patellar tendon, and hamstringsāneed time to adjust to this new geometry before they are asked to lift loads against gravity.
What to Do Instead: Your Action Plan for Weeks 1 to 4
Instead of aggressive strengthening right now, your primary focus should be calming the tissue down, restoring comfortable movement, and reducing protective muscle tension.
* Ease Up on Straight Leg Raises: If straight leg raises are triggering a sharp pinch behind the kneecap, give them a short break. Pushing into sharp pain at day 8 is unnecessary and will only heighten muscle guarding.
* Prioritize Pain-Free Mobility: Focus on gentle, supported range-of-motion exercises (like heel slides within a comfortable, pain-free window) to keep the joint lubricated without overloading the patellar tendon or quadriceps mechanism.
* Incorporate Gentle Soft Tissue Work: Use light, soothing massage techniques around the muscles above the kneecap (the quadriceps) and the calf muscles to help coax the nervous system into letting go of excess tension. Avoid pressing directly on fresh surgical incisions or acutely tender joint lines.
* Shift to Strengthening Later: As a general guideline, allow the acute inflammatory and tension-release phase to settle (typically around week 5, or whenever your leg can move through exercises without sharp cramping or pinching) before reintroducing targeted strengthening.
How is your swelling and overall range of motion feeling outside of this specific exercise?

Are you currently waiting for physiotherapy through the National Health system? Did you know that waiting times can stretch up to 18 weeks, and that you might only get a single session before being left un-reviewed for another six to eight weeks?
What happens when that single appointment finally rolls around? Often, you're handed a generic sheet of exercisesāmoves that your muscles might not even be ready for yet because you are still in pain and inflamed.
As a physio who spends my free time paddleboarding, surfing, and strength training, I see this pattern all the time. When you're hurting, jumping straight into a random sheet of exercises without proper hands-on guidance can backfire.
Sometimes, one targeted private physiotherapy session at the right time and in the right place can make a massive difference. You don't have to cancel your National Health referral, but you can use private care strategically to bridge the gap.
Here is why early intervention changes everything:
* Bypass the compensation cycle: Get hands-on treatment to calm down acute pain right now, keeping you functioning, sane, and moving freely.
* Prepare your tissue: Make sure your muscles are actually ready to load before you start a strengthening program.
* Arrive ready: When your public sector appointment finally comes through, your body is already prepped, out of pain, and ready to take on advanced progression.
Don't wait around in discomfort or let small niggles turn into major compensation roadblocks. Get the right advice, hands-on treatment, and progression when you need it most.

After a long day of hunching over a laptop or looking down at a phone, your body is craving this. š
This simple 60-second "heart opener" using a rolled bath towel is one of the kindest things you can do for your postureāand your nervous system.
Here is why it works so well:
Physical Release: It gently counteracts the rounded-shoulder posture of "desk life," creating space across your chest and mobilizing your upper back.
Nervous System Reset: When weāre stressed, our bodies curl inward (a protective response). By opening your chest, you signal to your brain that it is safe to shift out of "fight-or-flight" mode and into a state of calm.
How to do it:
* Roll up a bath towel tightly.
* Place it horizontally behind your mid-back (right at the bottom of your shoulder blades).
* Lie back over it. Keep your knees bent and feet flat.
* Let your arms fall open to the sides, palms facing up.
Breathe slowly and allow gravity to do the work. Stay for 60 seconds (or up to 3 minutes if it feels good).

My physio sessions are different.
I approach you through the nervous system. Your muscles are seen as modulators of pain and nervous system regulation. I donāt just release pain through painful trigger points where you have to endure the discomfort. I work directly with the nervous system where pain is modulated throughout the entire body.
I listen to your body and your soul.
If you come into my treatment space with an openness to communicate, I receive the messages your body is sharing, and the treatment becomes fully intuitive. I love when that happens. It is as if I step out of the way and your system takes control. I release tension in places you never even realized you were holding it, and the moment it softens, a profound sense of calm and relaxation takes over.
This is the level of deep listening in an intuitive session. I don't decide what happensāyour body does.
Yes, I am a physiotherapist and I conduct a full clinical assessment, but my physio skills and techniques are directed entirely by your soulās guidance. Energy healing is infused throughout the treatment, always with your consent. Everything unfolds exactly as your inner being wishes.
If your soul feels you would benefit more from a standard, less intuitive approach, that is exactly what you will receiveāand that is perfect, too.
Intuitive physios are rare. But when you find one, you are finally seen, heard, and met on every level: body, mind, and soul.
