How do you dose a 15 mg/ O.5 ml vial of Zepbound. Let’s simplify this because I know the math can make people freeze. 👀

If you’re prescribed the 15 mg / 0.5 mL vial through Lilly Direct (by Eli Lilly and Company), you can absolutely use it as a multi-dose vial when titrating.

Here’s how it breaks down:

First — understand the concentration

15 mg in 0.5 mL
= 30 mg per 1 mL

If you’re using a U-100 insulin syringe:
• 100 units = 1 mL
• 1 unit = 0.01 mL

So we convert mg → mL → units.



Starting Dose: 2.5 mg

2.5 mg ÷ 30 mg/mL = 0.083 mL
0.083 mL = 8 units

✔️ Draw to 8 units for a 2.5 mg starting dose.



Titration Cheat Sheet

• 2.5 mg = 8 units
• 5 mg = 17 units
