I am a board certified geriatric psychiatrist offering online individual and group coaching to help adult children navigate the transition from child to cargiver.
Most adult children don’t find out how important power of attorney is...
Until the moment they desperately need it.
And by then... it’s too late to get it.
Here’s what this one document actually does:
Without it...
🚫 Doctors cannot legally discuss your parent’s condition with you
🚫 Banks cannot let you manage their finances
🚫 Insurance companies will not work with you
🚫 Facilities will not take direction from you
The entire machinery of caregiving runs through this one piece of paper.
And here’s the truth most families never face until a crisis forces them to:
Most parents don’t have one.
Not because they refused outright.
But because the conversation got stuck.
You tried to bring it up.
Your parent got quiet... defensive... changed the subject.
You backed off.
You tried again six months later.
Same result.
And eventually... the conversation just stopped happening.
Until the fall.
Until the hospitalization.
Until the moment you’re standing in a hospital hallway being told you cannot legally make a single decision for the parent you have been caring for your entire adult life.
That moment is preventable.
But only if you understand what’s actually making the conversation so hard...
And how to have it differently.

I want to say something that I wish someone had told me earlier in my career.
The families who navigate a parent’s decline with the least amount of chaos... the least amount of conflict... the least amount of regret...
Are not the ones who loved their parents more.
They are the ones who had one conversation before they needed to.
The power of attorney conversation.
And I know what you’re thinking.
I’ve tried. My parent shuts down every time I bring it up.
I hear this every single week. And what I want you to understand is that your parent is not shutting down because they don’t trust you.
They are shutting down because every time you bring it up... they are being asked to sit with three of the most uncomfortable things a human being can face.
The reality that they will not always be able to speak for themselves.
The Aging Parent Coach speak for yourself” instead of “we need to talk about power of attorney”...
Most parents will actually engage.
Because you are no longer asking them to face their decline.
You are asking them to protect their future.
Once you have that conversation... the rest is manageable.
Make sure the document is durable so it holds up when your parent needs it most.

Your parent agreed to do the power of attorney.
That was the hard part.
But here’s where most families make a mistake that completely defeats the purpose...
They get the wrong document.
And they don’t find out until a crisis... when it’s too late to fix it.
Here’s what you actually need to know:
Not every power of attorney is the same.
The most important word you are looking for is durable.
A durable power of attorney stays in effect when your parent loses capacity.
A general power of attorney? It automatically ends the moment your parent cannot make decisions for themselves.
Which is exactly when you need it most.
Families set up a general power of attorney... file it away... and then discover in the middle of a medical crisis that the document they were counting on is legally void.
Do not let that be your family.
There are also two types you need, not one.
A healthcare power of attorney lets you make medical decisions.
A financial power of attorney lets you manage money... accounts... and property.
Most families need both. Some assign them to different people based on who is better suited. That is completely fine.
Just make sure both exist. And that both are durable.
And one more thing most families skip entirely:
Name a successor.
If you are the primary agent... who is the backup if something happens to you?
Most state forms have a place for this.
Use it.
Without a successor... you have a single point of failure built directly into the document.
Kevin came to me after his father’s hospitalization having discovered their power of attorney was not durable.
Starting over in the middle of a crisis... under time pressure... is one of the most stressful things a family can go through.
“Working with Barbara gave me clarity and tools I didn’t even know I needed.”
— Kevin L., 50, Engineer
Getting the right document the first time matters more than most families realize.

The power of attorney conversation isn’t failing because your parent doesn’t trust you.
It’s failing because of the first sentence.
The moment you say “we need to talk about power of attorney”...
Your parent hears something that has nothing to do with paperwork.
They hear...
You are declining. And we need to prepare for it.
And everything shuts down.
But what if you never mentioned power of attorney at all?
What if you just asked your parent what they wanted?
Who they would trust to speak for them.
How they would want their wishes protected.
What matters most to them if something unexpected happened.
That is a conversation most parents will actually have.
Because it has nothing to do with decline.
It has everything to do with their voice being protected.

Every aging parent I have ever sat with in my office...
Wants the same things.
To maintain control.
To be respected.
To be heard.
To not be a burden.
To die with dignity.
To be remembered as the person they used to be... not the person they are becoming.
And not one of them has ever said...
“I don’t trust my daughter.”
So when your parent resists the power of attorney conversation...
They are not saying they don’t trust you.
They are saying...
I want to stay in control.
I don’t want to admit I am declining.
I don’t want this to be the moment everything changes.
And here’s where most adult children get it completely wrong...
They keep trying to convince their parent to trust them.
But trust was never the issue.
Here’s the reframe that changes everything:
Power of attorney is not about your parent giving up control.
It is about your parent seizing control.
It is your parent saying...
If something happens to me... here is exactly who speaks for me. Here is exactly what I want. Here is how I make sure my wishes are followed... and not left up to a doctor or a court or a hospital who has never met me.
That is not surrender.
That is the ultimate act of self-determination.
And when you frame the conversation that way...
Most parents will actually have it with you.
Because now it’s not about decline.
It’s about their voice being protected.
And that... is something every parent wants.

You’ve tried to have the power of attorney conversation.
Maybe more than once.
And every single time... your parent shuts down.
You’ve told yourself it’s because they don’t trust you.
You’ve told yourself they’re being stubborn.
You’ve told yourself they just don’t want to deal with it.
But here’s what is actually happening inside your parent’s head every time you bring it up:
They are being asked to think about their own death.
That is what power of attorney actually is to them.
Not a practical document.
Not a responsible plan.
A death and decline document that says...
when I cannot make my own decisions... someone else will.
Bringing it up forces your parent to imagine themselves in a state of incapacity.
To imagine themselves not being themselves anymore.
And that is one of the most threatening thoughts a human being can have.
So the brain does what brains do.
It avoids. It deflects. It changes the subject.
And then there’s the second layer...
The conversation feels like another loss of control.
Your parent has already been quietly watching themselves change.
They’ve watched friends decline.
They’ve noticed their body slowing down.
They’ve caught themselves forgetting words.
They are already... maybe terrified... that they are losing themselves.
Signing a power of attorney feels like saying out loud...
I am not the person I used to be.
Of course they resist.
The resistance isn’t stubbornness.
It isn’t about you.
It isn’t even rational.
It’s existential.
And once you understand that...
You stop taking it personally.
And you start having a completely different kind of conversation.

You brought it up.
You thought you were being responsible. Thoughtful. Proactive.
And your parent shut down.
Got quiet. Changed the subject. Said they didn’t want to think about it.
And you walked away feeling the same thing almost every adult child feels after that conversation:
“My mother doesn’t trust me.”
“My father thinks I’m after his money.”
“They would rather give it to a stranger than me.”
So you went quiet too.
You stopped bringing it up.
You told yourself you’d try again later.
And later turned into months. And months turned into years. And the document still doesn’t exist.
Here’s what I need you to hear as a psychiatrist who has sat with hundreds of aging parents and their adult children:
The resistance to power of attorney is almost never about trust.
Not even close.
Your parent is not shutting down because they doubt you.
They are shutting down because of something else entirely...
Something that has nothing to do with you...
And everything to do with what that conversation is actually asking them to face.
Once you understand what’s really happening inside your parent’s head...
The conversation stops feeling like rejection.
And starts feeling like something you can actually navigate.
