Realistic results in thick-skinned rhinoplasty are about refinement, not perfection.
The underlying cartilage and bone can be reshaped significantly, but the thick soft-tissue envelope may limit how sharply those changes become visible. Tip definition, in particular, is often softer and less dramatic, while swelling can persist for considerably longer.

TUESDAY TEACHINGS:
A bilobed flap is a technique in which we use nearby skin to cover a defect on the nose.
The first lobe: Nearby skin is rotated into the defect on the nose.
The second lobe: Skin next to the first lobe moves to cover the area from where the first lobe was taken.
Both lobes remain attached to their blood supply while they are moved into position.
